Documented History of Present Illness Information Displays Incorrectly

The IssueNextGen Healthcare has identified an issue on the Reason for Visit template where, when the user enters generic History of Present
Illness (HPI) information for the sixth reason for visit, the data does not display in the grid on the *Intake and *SOAP templates or on the Master document.ExampleIn this example, the user creates a new patient or opens an existing patient and creates a new encounter.
The user navigates to the *Intake template and selects a specialty and visit type.
The user scrolls down to the Reason for Visit panel, selects the Additional/Manage active text link, and launches the Reason for Visit template.
The user selects five reasons for visit, free texts the sixth reason for visit, and clicks Save & Close.
From the Reason for Visit panel, the user selects each reason for visit, launches the associated generic HPI template, documents information into the HPI field, and clicks Save & Close.
For the sixth reason for visit, the user notes that the Generic Free Form1 – HPI template launches instead of the Generic Free Form6 – HPI template.
The user documents information into the HPI field, clicks Save & Close, and sees that the documented HPI information does not display in the grid.
The user selects the sixth reason for visit again, the Generic Free Form1 – HPI template incorrectly launches.
The user scrolls down and generates the Master Document from the Generate Document button.
The user sees that the information is not displaying in the Master document.Screen 1: This screen shows the Reason for Visit template.
Note that free text is entered as the sixth reason for visit. Screen 2: This

Note that information is documented into the “HPI” field. Screen

Actions Required
Currently, there is no workflow workaround for this issue.
Users may utilize NextGen Template Editor to
edit the Reason for Visit template.
Below are the steps to create a custom template:
- Using template editor, follow the detailed instructions in the Maintaining a Default Database document found here to open the Reason for Visit template.
- Do not modify NextGen Healthcare templates; instead copy the original template and rename it using the Shared Fields option and select Yes when prompted to Update Template

Below are the steps to modify the custom template:
- Open a copy of the Reason for Visit template.
- Right-Click the Save & Close button and select Triggers.
The Triggers page displays.
- Double-click the second last trigger with the {LEFT( {HOPI_.popUpName6}, 16 )} = "hpi_generic_free" condition. The Trigger Info template displays.
- Click the Parameter button.
The Stored Procedure template displays.
- Change the value of the @cc_number parameter from 1 to 6 and click OK.
- Click Update > OK.
- After making the required changes, save the template.
Status
This issue will be hotfixed for clients on NextGen KBM v.8.3.8. Clients who are experiencing this issue can
link their practice and be kept up to date on the status of this issue on the Client Support Center website (http://csc.nextgen.com).
