Reducible Information Does Not Display on the Physical Exam Grid and Master_IM Document

The Issue

NextGen Healthcare has identified an issue on the *SOAP template where, when the user documents the reducible information in the Abdominal Hernia – PE template, it does not display in the Physical Exam grid and the Master_IM document.


Example

In this example, the user creates a new patient or opens an existing patient and creates a new encounter. The user navigates to the *SOAP template and selects a Specialty and Visit Type. The user scrolls down to the Physical Exam panel, selects Abdomen from the list, and launches the Abdomen – Physical Exam template. In the Specific positive findings section, the user selects the Positive option for Hernia. The user clicks the Detail active text link that launches the Abdominal Hernia – PE template. The user enters information in the Abdominal Hernia – PE template and selects the No option in the Reducible field. The user clicks Save & Close in the Abdominal Hernia – PE and Abdomen – Physical Exam templates.

The user sees the documented reducible information is not displaying in the Physical Exam grid. The user scrolls down and generates the Master Document from the Generate Document button. The user sees that the information is not displaying in the master_im document.

Actions Required

Until this issue is fixed, users should document reducible information in the Comments section.

Users also can utilize Template Editor to edit the affected templates. Below are the steps to create the custom templates:

  1. Using Template Editor, follow the detailed instructions in the Maintaining a Default Database document found here to open the pe_abd_hernia template.
  2. Do not modify NextGen Healthcare templates; instead copy the original template and rename it using the Shared Fields option and select Yes when prompted to Update Template Links.
  3. Select Button001 from the Fields List, right-click and select Triggers… to launch the Triggers dialog box.
  4. Select the trigger with the following conditions:

    Parameter:
    pe_abd_hernia_.sum_neg = “Reducible”

    Event: Button Click
    Action: Assign Field Value
    Condition: {pe_abd_hernia_.reducible} = “N”

  5. Double-click the trigger and launch the Trigger Info dialog box.
  6. Click on the ellipsis and launch the Assign Field Value dialog box.
  7. Double-click in the grid and launch another Assign Field Value dialog box and change the value in the Value Calculation field to:

    pe_abd_hernia_.sum_neg = “Non reducible”

  8. Click OK.
  9. Click the Add button to launch the Assign Field Value dialog box.
  10. Select  pe_abd_hernia_.sum_pos and add the following value in the Value Calculation field:

    {IFELSE( {pe_abd_hernia_.sum_pos} != "", ({pe_abd_hernia_.sum_pos} + "." + " " + "Non reducible"), "Non reducible")}

  11. Click the Add button and close the dialog boxes.
  12. After making the required changes, update and save the template.
  13. Follow the detailed instructions in the Maintaining a Default Database document found here to open the pe_abdomen template.
  14. Do not modify NextGen Healthcare templates; instead copy the original template and rename it using the Shared Fields option and select Yes when prompted to Update Template Links.
  15. Select either pe_abdomen_.hernia: “Hernia” or pe_abdomen_hernia: “” from the Fields list and select Triggers… to launch the Triggers dialog box.
  16. Select the trigger with the following conditions:

    Parameter:
    pe_abd_hernia_.comments = “”

    Event” item selected
    Action: Assign Field Value
    Condition: {pe_abdomen_.hernia}!=1

  17. Double-click the trigger and launch the Trigger Info dialog box.
  18. Click on the ellipsis and launch the Assign Field Value dialog box.
  19. Click the Add button to launch another Assign Field Value dialog box.
  20. Select pe_abd_hernia_.sum_pos and add an empty value in the Value Calculation field by typing in “”.
  21. Click the Add button and close the dialog box.
  22. Click the Add button to launch another Assign Field Value dialog.
  23. Select pe_abd_hernia_.sum_neg and add an empty value in the Value Calculation field by typing in “”.
  24. Click the Add button and close the dialog boxes.
  25. After making the required changes, update and save the template.
   
Status

This issue will be hotfixed for clients on NextGen KBM v.8.3.8.
