Coding Points May Not Be Assigned for Groin, Buttocks, or Genitalia on SOAP – Dermatology Template

The Issue

NextGen Healthcare has identified an issue on the SOAP - Dermatology template where a coding point is not being assigned when groin, buttocks, or genitalia are documented as being examined.

Example

In this example, the user creates a new patient or opens an existing patient and creates a new encounter. The user navigates to the Home Page - Dermatology template and selects Office Visit as the visit type. The user selects the SOAP tab, scrolls down to the Areas Examined section of the Physical Exam panel. The user selects the Groin, Genitalia, or Buttocks check boxes and clicks Save. The user sees the documented information is displayed on the grid.

The user selects the Finalize tab, ensures the Established patient radio button is selected, and scrolls down to the Evaluation and Management Coding panel. The user selects a Medical Decision Making radio button and clicks Calculate Code. The user sees the coding point was not assigned on the 97 Single System Physical Exam Details, the 95 Physical Exam Details, or the 97 Multi System Physical Exam Details panels.

Currently, there is no workflow workaround for this issue.

Users may manually map the bullet ID on the Ngkbm EM Config template.

Below are the steps for the 97 Dermatology Single System Exam guidelines:

  1. Navigate to File > System / Practice Template.
  2. Click the All radio button on the System tab and open the Ngkbm EM Config template.
  3. Navigate to “Page 2 (mapping)”. Clicking on the words will move it to the correct section of the template.
  4. Select SS_Skin in the EM Type drop down list and pe_skin in the Table drop down list.
  5. Click chk_Buttocks in the Mapping grid and select ss_skin_4_genitalia/groin/buttocks from the Pick a Bullet ID drop down list.
  6. Repeat this process for the chk_genitalia_1 and chk_groin items in the Mapping grid (assign the same ss_skin_4_genitalia/groin/buttocks bullet ID).
  7. Navigate to page 3 and confirm they are displaying with these values (highlighted lines on screen shot below). This should be automatic, but the user should confirm they are correct.
  8. Click Apply Changes.
  9. Close the System Template.

Below are the steps for the 95 Exam guidelines:

  1. Navigate to File > System / Practice Template.
  2. Click the All radio button on the System tab and open the Ngkbm EM Config template.
  3. Navigate to “Page 2 (mapping)”. Clicking on the words will move it to the correct section of the template.
  4. Select MS_95 in the EM Type drop down list and pe_skin in the Table drop down list.
  5. Click chk_Buttocks in the Mapping grid and select ba_ggb from the Pick a Bullet ID drop down list.
  6. Repeat this process for the chk_genitalia_1 and chk_groin items in the Mapping grid (assign the same ba_ggb bullet ID).
  7. Navigate to page 3 and confirm they are displaying with these values (highlighted lines on screen shot below). This should be automatic, but the user should confirm they are correct.
  8. Click Apply Changes.
  9. Close the System Template.

Below are the steps for the 97 Multi System Exam guidelines:

  1. Navigate to File > System / Practice Template.
  2. Click the All radio button on the System tab and the open Ngkbm EM Config template.
  3. Go to “Page 2 (mapping)”. Clicking on the words will move it to the correct section of the template.
  4. Select MS_All in the EM Type drop down list and pe_skin in the Table drop down list.
  5. Click on chk_Buttocks in the Mapping grid and select ms_skin_1 from the Pick a Bullet ID drop down list.
  6. Repeat this process on chk_genitalia_1 and chk_groin items in the Mapping grid (assign the same ms_skin_1 bullet ID).
  7. Navigate to page 3 and confirm they are displaying with these values (highlighted lines on screen shot below). This should be automatic, but the user should confirm they are correct.
  8. Click Apply Changes.
  9. Close the System Template.

Status

This issue will be hotfixed for clients on NextGen KBM v.8.3.8. Clients who are experiencing this issue can link their practice and be kept up to date on the status of the issue via the Client Support Center website (http://csc.nextgen.com). Just navigate to the Known/Fixed Issues tab, select the affected product from the navigation bar and filter by the Issue ID.
